Who is eligible?
Available to all residents under age 65 living in AL, AK, AZ, AR, CA, CO, CT, DE, DC, FL, IL, IN, IA, KS, KY, LA, MD, MI, MS, MO, NE, NC, ND, OH, OK, SC, SD, TN, TX, UT, and WY. Not available to residents of HI, ID, MA, ME, MN, MT, NH, NJ, NM, NV, NY, OR, PA, RI, VT, VA, WA, WV or WI.

About the Insurance Company
UnitedHealthOne provides supplemental insurance benefit plans to individuals through Golden Rule Insurance Company. Plans are administered by Dental Benefit Providers Inc.

Product Review

Preventative Care is covered at 100% of prevailing rates without any deductible or waiting period.

Basic Services are covered at 80% of prevailing local rates include simple fillings, extractions, x-rays and treatment to ease dental pain.

Major Services are covered at 50% of prevailing local rates include root canals, crowns, inlays, onlays, veeners, bridges, dentures and oral surgery for impactions. These are covered after a 12 month waiting period.

Orthodontic care is not covered.

PPO Network includes 73,000 dentists nationally.

Out of network coverage is provided at the local reasonable and customary rate.

Maximum Benefit is $1,000 per person per year.

X-rays are covered once per year; no waiting period.

Vision coverage option - Optional vision coverage will help pay for eye exams with a $10 co-pay, frames and lenses, and contact lenses with a $25 co-pay. There is no deductible or waiting period for benefits. Like the dental benefit, the vision benefit offers better savings for in-network services. The vision care network includes 24,000 private practice and retail chain providers nationally.

Enrollment Support - Live personal enrollment support is available from the enrollment site weekdays from 8 AM to 6 PM Eastern time.

Pros

A large national dental PPO network is available in most urban areas to reduce premium costs and out-of-pocket fees.

Cons

Waiting periods make individual dental insurance less attractive to the majority of individuals who shop for coverage immediately before major dental services are required. As a result, only a small percentage of those who want to purchase dental insurance actually enroll in the coverage. For those who need immediate major dental care, a Careington discount dental plans provide a better alternative in the short term. 

When using an out-of-network dentist there is no assurance that the dentist will accept the insurer's calculation of usual and prevailing fee as full payment.
