This article is meant to help distinguish between an
online registration without insurance issued
(for example, when a quote is requested) and an actual
confirmation that coverage was issued on the
Core Health Insurance Web site.
After entering your name and email address online on the United States Fire
Insurance Company Core Health Insurance Web site the
plan administrator sends a confirmation email that included
the phrase "Registration Complete - Core Health Insurance". This email
can be misunderstood because it is not
a confirmation that coverage is in force.
A separate email is
sent to confirm that coverage is in force after completing
the entire application and authorizing an initial payment. In the event that
you begin an application online but do not complete it at
the same time (for example if you do not include a bank card
payment information) then the first e-mail will be sent but
not the second.
Core Health Insurance online registration confirmation
This is the email sent after you enter your name and
email on the Core Health Insurance Web site.

Coverage in force confirmation email
Following is the e-mail message sent after a policy is
approved and payment is received.
"On behalf of United States Fire Insurance Company, we are
pleased to inform you that your Core Health Insurance
application has been received, and your insurance ID number
is FLM000000.
We will be mailing your plastic Identification cards via
USPS within 1-3 business days. Your coverage is subject to
all policy provisions, conditions, limitations and
exclusions. If you have questions regarding your coverage
for specific services, please review your certificate of
insurance. This policy number does not guarantee that all
services will be covered.
This policy is ongoing and payments will continue to post until we
receive written notification of your request to cancel.
If any information you provided is incorrect or incomplete,
please immediately notify us by replying to this e-mail or
calling the Customer Service Department toll-free at
1-800-279-2290 (Mon-Fri 8:30 a.m. to 5:00 p.m. CST).
To access your application, certificate and id cards, sign
up now for online account access. For security purposes, we
do not email you your application or policy certificate. If
you opted to send in your payment, your certificate and id
card will be made available to you once your payment has
been processed. Your online account will allow you the
